Applications for School Year (2012-2013) are now underway. Application is on first come, first served basis. You may download the application form here. This form must be submitted in duplicate copy.
Note: Because of November 07, 2011 declared as Holiday, the High School Entrance Test is postponed to November 21, 2011.
1.1 Grade 6 Applicants (schools without Grade 7)
Average of 85 with no grade below 80
1.2 Grade 7 Applicants
Average of 82 with no grade below 80
1.3 Deportment average of 85 or B
2 recent colored ID pictures (1”x1”)
Certification from the School Principal that the applicant is a candidate for graduation.
Photocopy of Birth Certificate (NSO Copy)
Non-refundable testing fee of Php500.00
For non-Filipino citizens, photocopy of Alien Certificate of Registration (ACR).
For those with dual citizenship, photocopy of Philippine Passport or Certificate of Recognition as a Filipino Citizen.
Second Year Applicants
La Salle Green Hills accepts applicants only when slots are available.
A. Requirements:
Average of 85 with no grade below 80.
2 recent colored ID pictures (1”x1”).
Certificate of good moral character
Photocopy of Birth Certificate (NSO Copy)
Non-refundable testing fee of Php500.00
For non-Filipino citizens, photocopy of Alien Certificate of Registration (ACR)
For those with dual citizenship, photocopy of Philippine Passport or Certificate of Recognition as a Filipino Citizen.
Third Year Applicants
La Salle Green Hills does not accept third year applicants due to three year residency requirement for graduation.
Procedure
Submit all requirements together with the accomplished application forms to the Registrar’s Office.
*Application forms can be secured from the Registrar’s Office or can be
downloaded from the LSGH website. www.lsgh.edu.ph.
Pay the testing fee of Php500.00 at the accounting office.
Submit the proof of payment to the Registrar’s Office and get testing permit.
Reminders 1. If grades are not in numerical system, have them translated for accurate evaluation,
e.g. B=85%
2. Those who do not meet the above requirements need not apply.
3. Application is on a FIRST COME FIRST SERVED basis.
Exam Dates: Testing date will be given upon application.
Release of Test Result: Result of the entrance test will be e-mailed to you and uploaded in our website
www.lsgh.edu.ph. Inquiries regarding test result will not be entertained over the phone.
