Applications for School Year (2011-2012) are now underway. Application is on first come, first served basis. You may download the application form
here. This form must be submitted in duplicate copy.
Requirements and Prerequisites:
- Submission of Certified True Copy of recent F138 (Report Card)
- Grade 6 Applicants (schools without Grade 7) - average of 85 with no grade below 80.
- Grade 7 Applicants - average of 82% with no grade below 80.
- Deportment average of 85 or B
- 2 recent colored ID pictures (1" x 1")
- Recommendation letter from the applicant's Principal (It must be indicated that the applicant is graduating from elementary course.)
- Photocopy of Birth Certificate
- Payment of testing and registration fees (Php 500.00)
- For Expatriates - Submission of Study Permit/Verified Photocopy of ACR
Special Notes:
- If grades are not in numerical system, have them translated for accurate evaluation (e.g. B=85%).
- Those who do not meet the above requirements need not apply.
